Content Policy

This service is provided by Mindtra Health, LLC ("Mindtra"). MINDTRA DOES NOT PROVIDE ANY MEDICAL OR PROFESSIONAL SERVICES. Mindtra operates web-based and native platforms ("the App") to store and transmit requests for mental health services to licensed psychiatric providers and therapists ("Healthcare Providers"). Once a patient (or legal guardian of a patient) initiates a request, Mindtra enables connection with licensed providers who may offer services in the patient's state. These providers are third-party beneficiaries of this agreement and are not employees or direct affiliates of Mindtra Health, LLC.

Mindtra does not discriminate on the basis of race, color, national origin, age, marital status, sex, sexual orientation, gender identity, gender expression, disability, religion, or veteran status.

Patient Rights

As a patient or legal representative, you have the right to participate in treatment voluntarily, and to withdraw consent at any time, resulting in termination of services through Mindtra. You are entitled to receive respectful, confidential care in a secure virtual environment. You have the right to be informed clearly and fully about your diagnosis, treatment options, risks, and alternatives. You may identify any confusion, raise safety concerns, and know the identity and credentials of your provider. You may request provider changes, access your medical records, receive billing explanations, and submit complaints or feedback without fear of retaliation.

Not for Emergencies

Mindtra is not a resource for emergencies. In a medical or psychiatric emergency, dial 911 or visit the nearest emergency room. For urgent mental health crises, dial 988 or contact the Crisis Text Line by texting 741741.

Telehealth Services and Limitations

Telehealth involves care through secure, remote electronic communications. By using Mindtra, you understand that in-person assessments and diagnostics may be limited. Your provider may determine that a telehealth consultation is inadequate or not appropriate for your needs, which may necessitate in-person care. No treatment outcome or benefit can be guaranteed.

Establishing a Provider-Patient Relationship

You understand that a provider-patient relationship is only formed after completion of your intake, initial consultation, and acceptance of your provider. Submitting a request or payment alone does not establish a duty of care. Your provider may discontinue care or refer you elsewhere if telehealth is deemed inappropriate or if noncompliance or disrespectful behavior is exhibited.

Scope of Care

Mindtra facilitates care exclusively for treatable mental and emotional health disorders as determined by your provider. Other unrelated healthcare needs must be managed through alternative medical services.

Reliance on Provided Information

Your provider will rely on the information you provide to determine treatment safety and appropriateness. Inaccurate, incomplete, or misleading disclosures may increase health risks and lead to termination of care. Mindtra and your provider are not liable for consequences resulting from untrue or omitted information.

Treatment Plan and Adherence

Your provider may prescribe medication or therapy based on an agreed treatment plan. You understand that adhering to recommendations—including taking medications, attending sessions, and completing assigned tasks—is essential to achieving positive outcomes. Failure to comply may result in discontinuation of services.

Risks and Benefits

Treatment via Mindtra may include risks such as adverse medication reactions or emotional distress during therapy. You are encouraged to discuss alternatives—including lifestyle changes, support groups, or supplements—with your provider.

Communication and Technology Limitations

Real-time communication is limited to scheduled video sessions. Asynchronous messages are answered within one business day, though delays may occur. You are responsible for checking your App and email for updates. By providing your phone number, you consent to receiving calls and text messages for clinical communications.

Controlled Substances

Controlled substances may be prescribed only at the discretion of the licensed medical provider, in accordance with applicable federal and state laws. The provider will determine, based on your clinical presentation and medical history, whether a controlled substance is appropriate and necessary for your treatment. In some cases, an in-person medical evaluation may be required prior to prescribing certain controlled substances. Additionally, follow-up visits, including monthly or more frequent check-ins, may be required to ensure safe and effective monitoring. Not all providers within Mindtra's network prescribe controlled substances, and prescriptions are not guaranteed as part of your treatment plan.

Accommodation Requests

Providers are not obligated to complete disability or accommodation forms. Any such decision is at the provider's sole clinical discretion. Mindtra does not support requests related to emotional support animals, long-term disability, workers' compensation, or court-mandated evaluations.

Use of Technology and Electronic Health Risks

Mindtra may use technology tools to support documentation and care delivery. All data will be handled in accordance with HIPAA standards. You understand that electronic transmission carries inherent risks despite safeguards in place.

Health Information Exchanges (HIEs)

Mindtra may share your information with HIEs or Health Information Networks to improve coordinated care. You may opt out by contacting compliance@mindtrahealth.com.

Assignment of Benefits and Billing

By submitting your insurance, you authorize Mindtra to bill your health plan and release necessary information for claim processing. You are responsible for any charges not covered by insurance.

Informed Consent Confirmation

By using Mindtra's App and Services, you confirm that you have read, understood, and agree to this Informed Consent for Treatment. You understand your rights and responsibilities and voluntarily consent to receive mental health treatment via telehealth from licensed providers affiliated with Mindtra.
